Our long-term residential health care program is designed to address the needs of patient populations who require different levels of care, whether they function with a greater degree of independence, need assistance with activities of daily living; or require medical attention for conditions that are clinically complex.
In addition to providing traditional nursing home care, our facility has separate Alzheimer’s Dementia and sub-acute units specializing in post-hospital, short-term rehabilitation.

Pinnacle Multicare’s rehabilitation center features a television area, as well as a model kitchen, living room, bed and even a car. Residents can practice using these items under the supervision of our rehabilitation team before going home to ensure a smooth transition back into day-to-day life.
Our rehabilitation staff consists of licensed Physical and Occupational Therapists, Therapy Aides and Speech Pathologists. All services and modalities are provided in the state-of-the-art rehabilitation suite with modern equipment and supplies. Therapy is hands-on; residents are provided with individual attention throughout their session.
Pinnacle Multicare Center offers therapy six days a week, including evening and weekend sessions to provide the most convenient treatment program possible for residents.

In-Patient Unit Hospice Services
Pinnacle Multicare Nursing & Rehabilitation Center is one of the first dedicated hospice programs within a nursing home in the Bronx. We have come together with the Hospice of New York to provide palliative and supportive care to meet the special needs arising out of physical, psychological, spiritual, social and economic stresses which are experienced during the final stages of illness, and during dying and bereavement.

Respite Services
Respite services allows individuals with special needs to enter the facility for a temporary stay (from several days to three weeks) while their caregivers are away or taking temporary relief from the emotional and physical stresses which are often associated with caregiving.
All Respite guests reside in a beautifully appointed private room. During respite stay, each guest will receive individualized care and attention and a personalized care plan which addresses each guest specific care needs.
